All Applicants Must:
Be diagnosed with Lyme disease
Be age 26 or older
Be an active patient of a US-based Lyme-literate practitioner that has one of the following qualifications/titles:
Medical Doctor – MD
Naturopathic Doctor – ND or NMD (must have graduated from a CNME accredited institution. All practitioners must have prescribing rights in their state of practice.
Doctor of Osteopathy - DO
Physician Assistant - PA-C
Nurse Practitioner – NP
*NOTE: The following DO NOT currently qualify as lead/overseeing Lyme-literate practitioners: Acupuncturist, Chiropractor, Herbalist, Energy Worker, Magnet Therapist, Physical Therapist, Social Worker/Psychologist, Nutritionist, Health Coach
